Case Management and Advocacy
Most families do not know where to start when confronted with caring for a parent or disabled family member. That's where we come in. We have over 20 years of experience in helping families identify the resources needed to help their loved one, and we prepare specific care plans for families to follow to support elders or disabled family members. When needed, we either provide the support services directly or guide the family to resources that will meet the need. Often seniors or the disabled need a strong advocate to represent them in the health professions, government agencies, and the community. We are very effective advocates, well respected in the community, and knowledgeable of the systems that come into play in your loved one's life. We provide these services:
